1. Grants to organizations outside of Overlook Medical Center shall be made only for "such...hospital, health care or other medical purposes as the board of trustees may deem appropriate...to organizations tax exempt...under Section 501 (c)(3)...serving the health needs of citizens served by Overlook Medical Center. Funded projects must be consistent with the mission of Overlook Medical Center to provide exemplary health care services which are responsive to the needs of the community.
2. Preference is given to funding new programs and necessary equipment.
3. Grant proposals are accepted on a rolling basis and considered by the board quarterly.

Individual grants are limited to no more than $10,000.
4. Grant proposals must include the following information:
· Completed application form
· List of board members and their affiliations
· Tax letter
· Budget for the grant fiscal year including revenue and expenditures
· List of grant support from outside sources of the past two years
5. Grant applications must be submitted in the following format: (1) original (secured with a clip) and seventeen (17) copies of the full proposal stapled. We discourage the use of folders and binders.

6. The Foundation will require as condition of any grants, the following agreements by recipients:
a. the right of the Foundation to make the first announcement of grants on its behalf, or jointly, with the recipient; the right to review and modify with respect to Overlook's investment, recipient's subsequent announcement before release.
b. issuance by recipients of regular reports on progress of funded project: use of funds, comparison of results with objectives as stated in proposal.
